Incision under the breast
A very thin patient with virtually no breast tissue. The particular
issues for this type of patient are firstly, she is at high risk
of getting palpable or even visible rippling of the implant. This
is because there is so little of the patient’s own tissue
to cover the implant. The risks of rippling were minimised by
choosing silicone rather than a saline implant and also choosing
a CML style implant. This implant contains a stiffer than usual
gel that feels slightly firmer than standard gel but ripples less.
Rippling risks were further reduced by placing the implants under
the muscle. Secondly this type of patient cannot get a very natural
look, partly because the implant is so close to the surface, and
partly because people with this degree of thinness do not naturally
have full shaped breasts of this size. The gap between the breasts
is determined by the width of the implant chosen and the gap that
the patient has already. So to reduce the gap a larger implant
would have to have been chosen. However, the breasts are mobile
and with a bra the gap can be made smaller. A low profile implant
was selected to minimise “fakeness”. Had a high profile
implant been used an even rounder look would have resulted and
the gap would have been wider. Patients with no breasts and therefore
no breast fold worry that an incision under the breast will be
visible. There is no need for concern because, as you can see,
once the implant is in place a breast fold is created and this
hides the scar.
