Under the muscle with dual plane technique
Incisions under the breast
A difficult case. The patient has empty, sagging breasts with
thin stretched breast tissue. She was borderline for having a
breast lift but wished to avoid the extra scars of a lift. If
the implant is put behind the muscle in this type of breast, the
breast tissue may still hang down away from the implant, the so-called
“Snoopy” effect because it can look like Snoopy in
profile. This occurs because when the muscle is in between the
implant and the nipple the lifting “bucket handle effect”
is reduced. Therefore the nipple may still hang down. This can
be avoided by putting the implant in front of the muscle but,
in this patient, rippling might well then occur because the top
of the implant would only be covered by her thin, stretched breast
tissue. This patient’s best option was a special technique
called “dual plane”. This is where the implant ends
up with the top half covered by the muscle, ( reducing excessive
roundness and rippling) and the bottom half in front of the muscle
(allowing the bucket handle effect to “lift” the nipple
and avoid the Snoopy look).
